Primary Health Care
The Primary Health Care Program consists of a team of physicians, nurse practitioners, nurses and other allied health professionals who work in collaboration with other programs offered by Brock CHC. Services include: preventative health care, treatment of chronic and episodic illnesses, mental health counselling, service navigation and referrals, access to phlebotomy, and more. All Brock CHC services are free. At this time, our primary care providers are operating at full capacity and we are therefore unable to roster new clients. However, we have partnered with North Durham Family Health Team to run an Unattached Patient Program for people in Brock Township who do not have a primary care provider. Please note that residents of Brock Township and surrounding areas can still access our Physiotherapist, Social Worker, Dietitian, Diabetes Education Program, Geriatric Assessment Program and our Community Wellness Programs and Activities at any time.

If you would like to be added to our wait list, please complete the Request for a Primary Health Care Provider form which can be picked up in person, mailed, or downloaded. Our current wait time is 2 years.

Please note that Brock CHC is not a walk-in clinic. There are some Urgent Care Clinics in neighbouring areas.
