Geriatric Assessment
The Geriatric Assessment Program provides comprehensive assessment services for frail seniors, primarily aged 65 and older, living within Brock Township and surrounding areas (at home or in retirement residences) who have complex health care needs. A collaborative team approach is used to collaborate with other health services and community agencies in referring seniors for further assessment or health and social services with the goal of maintaining or improving health, promoting functional independence and quality of life, and preventing or delaying institutionalization, as appropriate. All Brock CHC services are free.
To access services, please complete the Geriatric Assessment Program Referral Form and send by fax (705.426.3330) or drop off in person.
Behavioural Supports
Behavioural Supports is a service provided at Brock CHC for people living with expression of responsive behaviours. Responsive behaviours are the means by which a person with dementia or other conditions may communicate an unmet need. If you think that you, or someone you care for, would benefit from an assessment of responsive behaviours and a trial of interventions, please give us a call. For examples of responsive behaviours or to complete a referral, please see the link to the Geriatric Assessment Program Referral Form above.
